Effectiveness of Measurement of Serum Testosterone Concentration as Screening Test in Inpotent Men

From Februarl 1993 to July 1993, serum testosterone concentrations were measured in 297 impotent men as a screening test. Libido was assessed by response to response to a questionnaire. Testis size was determined using an orchiometer, and atrophy
was
defined as a testicular volume less than 15mL. Of the 297 patients, 32(10.8%) had low serum testosterone concentrations, but on repeated testing only 9 patients(3.0%) had an abnormally low value. Five patients had primary hypogonadism and four
had
secondary hypogonadism. All 9 patients with documented endocrinopathy had decreased libido(7) or testicular atropohy(5). Based on this study, we recommend limiting endocrine screening of impotent men to those who have either decreased libido or
testicular atrophy.
